New Patient Registration Instructions
Why Use Our Patient Portal?
We are encouraging all families and patients to register for our practice portal as we are integrating it with every aspect of your experience at Heart to Heart Pediatrics. With portal access you can:
- register your child to become a patient at Heart to Heart Pediatrics
- check-in for all appointments with your phone or computer
- view and update personal health information
- ask our staff questions about your child
- request prescriptions
- receive test results
- view patient care summaries and office visit notes
- read patient education materials
- make payments on your account
- send questions about your bill or look at your payment history
All communications through the portal are HIPAA secure. We will be using the portal as the primary mode of communication for lab results and other updates. In addition, many of the handouts you are used to receiving in the office, including referral sheets and well-check patient education materials, will now be sent out via the portal.

Register for a Portal Account

Clicking the button will take you to our portal with Athena Health. After creating an account, please review and sign the new patient registration form under:
MY HEALTH ->MEDICAL FORMS.
Each child needs their own account, and we will be able to link all family members in the portal at your first visit.
Also, 3 days before your child’s first visit, make sure to fill out his/her past medical history through the appointment check-in link.
PATIENT PORTAL REGISTRATION INSTRUCTIONS
Registration can be done on a mobile device, but it is easier if done on a computer.
If you have more than one child, each child will need to be registered separately.
- Click Sign up.
- IMPORTANT!!!!! Under “Who will use this account” check Patient’s family member.
- Enter in your child’s information. You can use a parent’s email address. Phone can be a home line or a cell phone. Make sure to use parentheses with your number.
- Click Continue and the next page will send you a verification link to the email address or phone number listed.
- After verification, create your account password.
- Enter in parent’s information. (In order to link accounts, this must be the same information for all registered children.)
- You have created your child’s account.
We recommend completing 3 tasks in your child’s account:
- Click on the Upper Right Dropdown Box (next to your child’s name) and select My Profile. Verify all information is correct and add/edit any entries.
- Click on My Health on the left side. Click on Medical Forms on the top. Review the New Patient Packet.
- Then click and review the Acknowledgment of Office Documents. Enter you name in the box at the bottom of the form to E-sign the document
IF YOU HAVE MORE THAN ONE CHILD PLEASE READ CAREFULLY:
- Log Out of your first child’s account.
- Follow the same directions, starting with clicking Sign up.
- DON’T FORGET to check Patient’s Family Member for the other kids.
- Enter in the same e-mail address for all children.
- After signing up all your children, you should be able to switch between accounts by clicking the Upper Right Dropdown Box. If you cannot see all your kids, don’t worry, they were registered, however, they likely were not linked to the same portal account. We will take care of this when you call to schedule your first appointment with any of your kids.
